Description: Our client is currently seeking a Occupational Therapist - Home Health

Job Summary: Looking for a dedicated Occupational Therapist for Home Health in Arizona. As an Adult Occupational Therapist, you will conduct home visits to evaluate and treat adult patients, helping them regain independence in their daily activities. You will work closely with individuals facing physical, cognitive, or emotional challenges to develop personalized treatment plans and implement therapeutic interventions.

Responsibilities:

  • Perform comprehensive assessments of adult patients' functional abilities, including physical, cognitive, and emotional capacities.
  • Collaborate with patients, their families, and interdisciplinary healthcare teams to develop individualized treatment plans.
  • Provide evidence-based therapeutic interventions to address patients' needs and goals.
  • Conduct regular re-evaluations to track progress and modify treatment plans as necessary.
  • Assist patients in improving skills and abilities related to activities of daily living (ADLs), such as dressing, bathing, eating, and grooming.
  • Teach compensatory techniques and provide adaptive equipment to enhance independence and safety.
  • Educate patients and their families on injury prevention strategies and adaptive strategies for daily activities.
  • Document patient evaluations, treatment plans, progress notes, and discharge summaries accurately and timely.
  • Maintain professional competency through continuing education, attending workshops, and staying updated with the latest research and techniques.
Qualifications:
  • Bachelor's or master's degree in occupational therapy from an accredited program.
  • Current AZ license or eligibility for licensure as an Occupational Therapist.
  • Minimum of 2 years of experience working as an Occupational Therapist, preferably in adult home health.
  • Strong knowledge of adult anatomy, physiology, and common conditions affecting the adult population.
  • Proficient in conducting assessments and interpreting assessment findings to develop appropriate treatment plans.
  • Ability to work effectively in a team and independently, while managing multiple patients and priorities.
  • Excellent communication and interpersonal skills to interact with patients, families, and healthcare professionals.
  • Familiarity with electronic medical record systems and basic computer skills.
  • CPR certification (or willingness to obtain it).
Position Details:
  • Visits: 12-18 per week
  • Adult Patients
  • Familiar with Oasis SOCs, Supervisory and Routine visits
  • Mon-Fri, No weekends.
Compensation:
  • Mileage Reimbursement: $0.67 per mile after 30 miles per day

  • Glendale
